Added prototypes and includes from patch by Randolph Fritz.
Added more required includes and reduced other compile warnings.

Instrumented headers against multiple inclusion and for use from C++.
Moved includes in headers out of "C" scope.

Replaced most outdated BSD function calls with their posix equivalents, and cleaned up a few other platform dependencies.

Broke standard.h into rtio.h, rterror.h, rtmath.h, and rtmisc.h
