[Radiance-dev] Radiane MIME types

I am turning back to this project, having finally found a bit of time.  
I've updated my draft registration form. It can be downloaded from <http://students.washington.edu/rfritz/radreg/Radiance%20Registration%20Form.txt

Two questions only:

   1. Is the file header different on Windows because of the  
difference in newline convention?
   2. Is *Rendering with Radiance* still available from Booksurge?
